To prepare you for your appointment please read and fill out the following documents. You can bring the Client Form and the Indemnity Form with you to your appointment, or upload them via our booking page.
Please note that if you choose to bring these forms to your appointment, allow 90 minutes for your appointment as I will need to go through the forms to make sure it is safe to use and apply essential oils to your body.
